Miracle Recovery: From Paralyzed to Helping Others

Janne Kouri was told he would never walk again. Paralyzed from the neck down after a freak accident in the ocean five years ago, Kouri's entire world was completely turned upside down. But rather than give up, Kourani pushed through with the support of his wife and family, and turned his tragedy into a life's work.

Now, five years after he was told there was no hope for recovery, the 36-year-old can stand up on his own without a walker.

On August 5, 2006, Kouri, of Hermosa Beach, Calif., was playing beach volleyball with some friends when he ran down to the ocean to cool off in between games. He dove into the waves, crashed his head into a hidden sand-bar, and was instantly paralyzed from the neck down.

"I knew something really bad had happened because I couldn't move anything," said Kouri, who was 31 at the time of the accident. "There definitely was -- a moment there where I was thinking that that could be my last breath."

Fortunately an off-duty EMT pulled him ashore and rushed him to the hospital. Kouri was alive, but a doctor delivered devastating news to his then-girlfriend, Susan.

"[The doctor] looked me right in the eye and said, 'You need to be prepared for him never to walk again,'" Susan Kouri, 36, recalled. "I will never forget that."

Before the accident, Kouri had worked as a director of an online social network and was a force of nature. At 6' 4" and 285 pounds, he was the star defensive tackle on the Georgetown University football field with NFL prospects and was called "the general" by his friends because of his take-charge attitude.

With his spinal cord fractured in two places, Kouri spent two months in intensive care, developed pneumonia and nearly died twice.

"There were many times he came to me and said, 'You know, you don't need to do this. You don't need to be here,'" Susan said. "I promised him that as long as his heart and his mind stayed the same that I would love him."

That love would change everything, giving Kouri the determination to move forward with his life. As his health returned, the reality of his paralysis was grim and treatment options were bleak.

Cutting-Edge Research Renews Couple's Hope

Finally, the couple found their ray of hope. In their research and after months of Susan traveling around the country visiting rehab centers, they discovered Dr. Susan Harkema at the Frazier Rehabilitation Institute in Louisville, Ky.

Harkema helped develop a cutting-edge therapy known as "loco-motor training," which teaches the spinal cord how to control motor functions like walking, through repetitive motion. The late actor Christopher Reeve was among her first test subjects to utilize the training; the therapy has now helped hundreds of spinal cord injury victims.

"Dr. Harkema was the one person that told us, 'There is hope for recovery,'" Kouri said.

The couple moved to Louisville to work with Harkema.

"The first day I got there they put me up on the treadmill and I passed out in seven seconds. Shortly after, within days after, I was able to stay up for ten, 15 minutes, and then an hour," Kouri said. "Very quickly I started to get a lot stronger. My circulation improved, blood pressure improved, muscle strength."

After two to three months of training, Kouri had his first milestone -- a little toe wiggle.

"I started wiggling one of my toes, which was an incredible moment," he said. "And I said, 'You know what? I have a chance here."

(AP) ? Searchers found the body of a 4-year-old boy Wednesday in the river at an Arkansas Boy Scout camp where he was tagging along with his aunt last weekend.

A search team in a canoe spotted Caleb Linn's body in a tributary of the Spring River, according to the Fulton County Sheriff's office.

Caleb went missing Saturday at the 900-acre Kia Kima Scout Reservation near Hardy, Ark. Searchers in boats and on horseback, foot and four-wheelers scoured the valley for days before finding two shirts he had been wearing in the river on Tuesday afternoon. His body was found less than 24 hours later.

"As long as we have each other, it's OK," said Caleb's uncle, Chris Bruner.

Authorities narrowed their focus to the water late Tuesday and called off the ground search for Caleb. He had gone with his aunt and several other children to help clear storm debris from a bridge at the camp that his aunt was looking after while the regular caretaker was away.

When five of the children left to go back to the cabins around lunchtime, Caleb asked if he could follow. His aunt told him he could, but that he had to stick to the road, Fulton County Sheriff Buck Foley said. The last she saw of him before she turned around to resume her work, he said, was Caleb standing at the end of the bridge.

Hundreds of volunteers turned out to help in the search, and divers cleared debris and stuck an underwater camera beneath the bridge and under ledges where they couldn't get a clear view. Dry and unseasonably warm weather had lent hope to Caleb's chances of survival. Even after searchers found the shirts he was wearing the day he disappeared, Caleb's great-grandmother held out hope someone would find him alive.

"Just because they found the clothes doesn't mean we give up hope," Carolyn Truex said Tuesday evening.

Arkansas State Police spokesman Bill Sadler said searchers found Caleb's body in the river about a half-mile from the bridge where the boy was last seen. His body was to be sent to the state crime lab.

Family members, including Bruner, had fought their way along sandy trails and through thorny branches that would snap back with a thwack as they searched for Caleb on Tuesday. Another uncle, Joey Dawson, even swam to a small island in the middle of the river, just in case Caleb had gotten stranded there.

Caleb's mom and stepdad had travelled from their home in Springdale, 150 miles west of Hardy, and have been staying at one of the cabins while searchers looked for the boy.

Shortly after searchers found Caleb's body, his family started to grieve near the bridge where he was last seen. Bruner says they'll likely bring his body back to northwest Arkansas for a funeral there. In the meantime, a memorial fund has been set up to help Caleb's family pay for his funeral.

"We have a little bit of money, but it's nothing like what he deserves," Bruner said. "We'll do what we can with what we have."

Why Healthcare Is Different | The Health Care Blog

Working in the health care space has forced me to give up many hopes and expectations that I had a few years ago. Forgive me for being cynical (it?s an easy feeling to have following the country?s largest health IT conference, as I reported a month ago), and indeed some positive trends do step in to shore up hope. I?ll go over the redeeming factors after listing the five tough lessons.

1. The health care field will not adopt a Silicon Valley mentality

Wild, willful, ego-driven experimentation?a zeal for throwing money after intriguing ideas with minimal business plans?has seemed work for the computer field, and much of the world is trying to adopt a ?California optimism.? A lot of venture capitalists and technology fans deem this attitude the way to redeem health care from its morass of expensive solutions that don?t lead to cures. But it won?t happen, at least not the way they paint it.

Health care is one of the most regulated fields in public life, and we want it that way. From the moment we walk into a health facility, we expect the staff to be following rigorous policies to avoid infections. (They don?t, but we expect them to.) And not just anybody can set up a shield outside the door and call themselves a doctor. In the nineteenth century it was easier, but we don?t consider that a golden age of medicine.

Instead, doctors go through some of the longest and most demanding training that exists in the world today. And even after they?re licensed, they have to regularly sign up for continuing education to keep practicing. Other fields in medicine are similar. The whole industry is constrained by endless requirements that make sure the insiders remain in their seats and no ?disruptive technologies? raise surprises. Just ask a legal expert about the complex mesh of Federal and state regulations that a health care provider has to navigate to protect patient privacy?and you do want your medical records to be private, don?t you??before you rave about the Silicon Valley mentality. Also read the O?Reilly book by Fred Trotter and David Uhlman about the health care system as it really is.

Nor can patients change treatments with the ease of closing down a Facebook account. Once a patient has established a trust relationship with a doctor and obtained a treatment plan, he or she won?t say, ?I think I?ll go down the road to another center that charges $100 less for this procedure.? And indeed, health reform doesn?t prosper from breaking down treatments into individual chunks. Progress lies in the opposite direction: the redemptive potential of long-term relationships.

2. Regulations can?t force change

I am very impressed with the HITECH act (a product of the American Recovery and Reinvestment Act, more than the Affordable Care Act) that set modern health reform in motion, as well as the efforts of the Department of Health and Human Services to push institutions forward. But change in health care, like education, boils down to the interaction in a room between a professional and a client. Just as lesson plans and tests can?t ensure that a teacher inspires a child to learn, regulations can?t keep a doctor from ordering an unnecessary test to placate an anxious patient.

We can offer clinical decision support to suggest what has worked for other patients, but we can?t keep a patient from asking for a expensive procedure that has a 10% chance of making him better (and a 20% chance of making him worse), nor can we make the moral decision about what treatment to pursue, for the patient or the doctor. Each patient is different, anyway. No one wants to be a statistic.

3. The insurance companies are not the locus of cost and treatment problems

Health insurers are a favorite target of hatred by Americans, exemplified by Michael Moore?s 2007 movie Sicko and more surprisingly in the 1997 romantic comedy As Good as it Gets, where I saw an audience applaud as Helen Hunt delivered a rant against health maintenance organizations. A lot of activists, looking at other countries, declare that our problems would be solved (well, would improve a lot) if we got private insurers out of the picture.

Sure, there?s a lot of waste in the current insurance system, which deliberately stretches out the task of payment and makes it take up the days of full-time staff in each doctor?s office. But that?s not the cause of the main problems in either costs or treatment failures. The problems lie with the beloved treatment staff. We can respect their hard work and the lives they save, but we don?t have to respect them for releasing patients from hospitals without adequate follow-up, or for ordering unnecessary radiation that creates harm for patients, or for the preventable errors that still (after years of publicity) kill 90,000 to 100,000 patients a year.

4. Doctors don?t want to be care managers

The premise of health reform is to integrate patients into a larger plan for managing a population. A doctor is supposed to manage a case load and keep his or her pipeline full while not spending too much. The thrust of various remuneration schemes, old and new, that go beyond fee for service (capitation, global payment systems) is to reward a doctor for handling patients of a particular type (for instance, elderly people with hypertension) at a particular cost. But doctors aren?t trained for this. They want to fix the immediate, presenting complaint and send the patient home until they?re needed again. Some think longitudinally, and diligently try to treat the whole person rather than a symptom. But managing their treatment options as a finite resource is just not in their skill set.

The United Kingdom?host of one of the world?s great national care systems?is about to launch a bold new program where doctors have to do case management. The doctors are rebelling. If this is the future of medicine, we?ll have to find new medical personnel to do it.

5. Patients don?t want to be care managers

Now that the medical field has responded superbly to acute health problems, we are left with long-term problems that require lifestyle and environmental changes. The patient is even more important than the doctor in these modern ills. But the patients who cost the most and need to make the most far-ranging changes are demonstrating an immunity to good advice. They didn?t get emphysema or Type 2 diabetes by acting healthily in the first place, and they aren?t about to climb out of their condition voluntarily either.

You know what the problem with chronic disease is? Its worst effects are not likely to show up early in life when lifestyle change could make the most difference. (Serious pain can come quickly from some chronic illnesses, such as asthma and Crohn?s disease, but these are also hard to fix through lifestyle changes, if by ?lifestyle change? you mean breathing clean air.) The changes a patient would have to make to prevent smoking-related lung disease or obesity-related problems would require a piercing re-evaluation of his course of life, which few can do. And incidentally, they are neither motivated nor trained to store their own personal health records.

Hope for the future

Despite the disappointments I?ve undergone in learning about health care, I expect the system to change for the better. It has to, because the public just won?t tolerate more precipitous price hikes and sub-standard care.

There?s a paucity of citations in my five lessons because they tend not to be laid out bluntly in research or opinion pieces; for the most part, they emerged gradually over many hallway conversations I had. Each of the five lessons contain a ?not,? indicating that they attack common myths. Myths (in the traditional sense) in fact are very useful constructs, because they organize the understanding of the world that societies have trouble articulating in other ways. We can realize that myths are historically inaccurate while finding positive steps forward in them.

The Silicon Valley mentality will have some effect through new devices and mobile phone apps that promote healthy activity. They can help with everything from basic compliance?remembering to take prescribed meds?to promoting fitness crazes and keeping disabled people in their homes. Lectures given once in a year in the doctor?s office don?t lead to deep personal change, but having a helper nearby (even a digital one) can impel a person to act better, hour by hour and day by day. This has been proven by psychologists over and over: motivation is best delivered in small, regular doses (a theme found in my posting from HIMSS).

Because the most needy patients are often the most recalcitrant ones, personal responsibility has to intersect with professional guidance. A doctor has to work the patient, and other staff can shore up good habits as well. This requires the doctors? electronic record systems to accept patient data, such as weight and mood. Projects such as Indivo X support these enhancements, which traditional electronic record systems are ill-prepared for.

Although doctors eschew case management, there are plenty of other professionals who can help them with it, and forming Accountable Care Organizations gives the treatment staff access to such help. Tons of potential savings lie in the data that clinicians could collect and aggregate. Still more data is being loaded by the federal government regularly at Health.Data.Gov. ACOs and other large institutions can hire people who love to crunch big data (if such staff can be found, because they?re in extremely high demand now in almost every industry) to create systems that slide seamlessly into clinical decision support and provide guidelines for better treatment, as well as handle the clinic?s logistics better. So what we need to do is train a lot more experts in big data to understand the health care field and crunch its numbers.

Change will be disruptive, and will not be welcomed with open arms. Those who want a better system need to look at the areas where change is most likely to make a difference.

This first direct estimate of the number of light planets around red dwarf stars has just been announced by an international team using observations with the HARPS spectrograph on the 3.6-metre telescope at ESO's La Silla Observatory in Chile [1]. A recent announcement (eso1204 - http://www.eso.org/public/news/eso1204/), showing that planets are ubiquitous in our galaxy used a different method that was not sensitive to this important class of exoplanets.

The HARPS team has been searching for exoplanets orbiting the most common kind of star in the Milky Way red dwarf stars (also known as M dwarfs [2]). These stars are faint and cool compared to the Sun, but very common and long-lived, and therefore account for 80% of all the stars in the Milky Way.

"Our new observations with HARPS mean that about 40% of all red dwarf stars have a super-Earth orbiting in the habitable zone where liquid water can exist on the surface of the planet," says Xavier Bonfils (IPAG, Observatoire des Sciences de l'Univers de Grenoble, France), the leader of the team. "Because red dwarfs are so common there are about 160 billion of them in the Milky Way this leads us to the astonishing result that there are tens of billions of these planets in our galaxy alone."

The HARPS team surveyed a carefully chosen sample of 102 red dwarf stars in the southern skies over a six-year period. A total of nine super-Earths (planets with masses between one and ten times that of Earth) were found, including two inside the habitable zones of Gliese 581 (eso0915 - http://www.eso.org/public/news/eso0915/) and Gliese 667 C respectively. The astronomers could estimate how heavy the planets were and how far from their stars they orbited.

By combining all the data, including observations of stars that did not have planets, and looking at the fraction of existing planets that could be discovered, the team has been able to work out how common different sorts of planets are around red dwarfs. They find that the frequency of occurrence of super-Earths [3] in the habitable zone is 41% with a range from 28% to 95%.

On the other hand, more massive planets, similar to Jupiter and Saturn in our Solar System, are found to be rare around red dwarfs. Less than 12% of red dwarfs are expected to have giant planets (with masses between 100 and 1000 times that of the Earth).

As there are many red dwarf stars close to the Sun the new estimate means that there are probably about one hundred super-Earth planets in the habitable zones around stars in the neighbourhood of the Sun at distances less than about 30 light-years [4].

"The habitable zone around a red dwarf, where the temperature is suitable for liquid water to exist on the surface, is much closer to the star than the Earth is to the Sun," says Stephane Udry (Geneva Observatory and member of the team). "But red dwarfs are known to be subject to stellar eruptions or flares, which may bathe the planet in X-rays or ultraviolet radiation, and which may make life there less likely."

One of the planets discovered in the HARPS survey of red dwarfs is Gliese 667 Cc [5]. This is the second planet in this triple star system (see eso0939 - http://www.eso.org/public/news/eso0939/ - for the first) and seems to be situated close to the centre of the habitable zone. Although this planet is more than four times heavier than the Earth it is the closest twin to Earth found so far and almost certainly has the right conditions for the existence of liquid water on its surface. This is the second super-Earth planet inside the habitable zone of a red dwarf discovered during this HARPS survey, after Gliese 581d was announced in 2007 and confirmed in 2009.

"Now that we know that there are many super-Earths around nearby red dwarfs we need to identify more of them using both HARPS and future instruments. Some of these planets are expected to pass in front of their parent star as they orbit this will open up the exciting possibility of studying the planet's atmosphere and searching for signs of life," concludes Xavier Delfosse, another member of the team (eso1210 - http://www.eso.org/public/news/eso1210/).

[1] HARPS measures the radial velocity of a star with extraordinary precision. A planet in orbit around a star causes the star to regularly move towards and away from a distant observer on Earth. Due to the Doppler effect, this radial velocity change induces a shift of the star's spectrum towards longer wavelengths as it moves away (called a redshift) and a blueshift (towards shorter wavelengths) as it approaches. This tiny shift of the star's spectrum can be measured with a high-precision spectrograph such as HARPS and used to infer the presence of a planet.
